LSVT LOUD®: Speech Therapy for Parkinson’s Disease and Similar Conditions
Learn about the LSVT LOUD, who it can help, and how it differs from other speech therapy programs.

LSVT LOUD is an effective speech treatment for people with Parkinson's disease (PD) and other neurological conditions.  Named for Mrs. Lee Silverman (Lee Silverman Voice Treatment), a woman living with PD, it was developed by Dr. Lorraine Ramig and has been scientifically studied for over 25 years with support from the National Institute for Deafness and other Communication Disorders within the National Institutes of Health (NIH) and other funding organizations. LSVT LOUD trains people with PD to use their voice at a more normal loudness level while speaking at home, work, or in the community. Key to the treatment is helping people "recalibrate" their perceptions so they know how loud or soft they sound to other people and can feel comfortable using a stronger voice at a normal loudness level.

While LSVT LOUD treatment has helped people in all stages of PD, the majority of research has been on those in moderate stages of the disease. LSVT LOUD has also helped people with atypical parkinsonisms, such as progressive supranuclear palsy (PSP), and has recently shown promise for adults with speech issues arising from stroke or multiple sclerosis and children with cerebral palsy or Down syndrome. Beginning your work with LSVT LOUD before you've noticed significant problems with voice, speech and communication will often lead to the best results, but it's never too late to start. LSVT LOUD has the potential to produce significant improvements even for people facing considerable communication difficulties.

Outcomes Backed by Research Evidence

The extensive research support documenting the effectiveness of LSVT LOUD makes it unique among speech treatment approaches for people with PD.

This research began in 1987 when Dr. Lorraine Ramig met Mrs. Lee Silverman (pictured right) and her family. "If only we could hear and understand her" was the expressed wish of Mrs. Silverman’s husband and her adult children. This motivated Dr. Ramig’s life-long work to develop an effective speech treatment for people with PD, scientifically document its effectiveness, and implement the treatment into real-word clinical practice.

The research simply speaks for itself. When selecting a program to offer at my clinic, I reviewed the research and outcomes for LSVT LOUD as well as several other more recently developed programs with a similar concept; however, none had the strong evidence base that LSVT LOUD has.

Today, LSVT LOUD is considered the global “gold standard” for speech treatment for people with PD. Extensive research on LSVT LOUD has documented that people with PD show improvements in loudness and more variation in pitch while speaking that are maintained for at least two years post-treatment. Secondary improvements may include better articulation, changes in swallowing, improved facial expressions while talking and even speech-related brain changes.

LSVT LOUD Treatment Explained
LSVT LOUD is an intensive, one-on-one treatment delivered over one month’s time:
One-hour sessions
Four sessions per week
Four weeks in a row
Daily homework and carryover exercises

LSVT LOUD clinicians will help you establish a daily routine of "homework" from day one, in which you continue to complete a short 10-15 minute set of the LSVT LOUD exercises, on your own.

The goal is for individuals to "rev up" their stronger voice with daily practice so they can use it comfortably in daily communication. Further, this will become a life-long habit of practice, even after the 16 sessions of treatment are completed.

This concentrated approach is critical for the positive changes LSVT LOUD achieves, consistent with what studies on motor learning and neuroplasticity (the ability for your brain and nervous system to adapt and evolve) show is most effective.

The care and skill of a speech-language pathologist is of the utmost importance when working on vocal loudness to ensure healthy voice quality. Although the concept of “speak LOUD” may appear simple on the surface, your LSVT LOUD Certified clinician is essential to help you learn how to improve vocal loudness in a way that does not harm your voice.

LSVT LOUD is typically delivered via in-person sessions, but there is the option for LSVT eLOUD where treatment is delivered using real-time videoconferencing. The LSVT LOUD clinician would be in his or her clinic and the person with PD would be at his or her home. Some or all of the 16 sessions can be delivered in this manner. Another technology supported approach for LSVT LOUD involves using the LSVT Companion Client Edition for a portion of the treatment sessions. A research study has documented that when 7 of the 16 total sessions were completed using the LSVT Companion, outcomes were comparable at 6 months to all sessions being in-person. Note that LSVT LOUD is not offered in fewer, shorter or more spaced-out sessions which would not give you the results that correct LSVT LOUD treatment achieves. Only alterations that are documented with research, such as the LSVT eLOUD or the LSVT Companion, are acceptable.

A Unique Approach to Better Speech

Speech treatments for people with Parkinson's disease have historically been focused on articulation and speech rate and have been given at a low-intensity dosage (e.g., one or two sessions per week for some period of time). LSVT LOUD takes a very different approach, focusing on the singular target of increasing vocal loudness to normal levels and delivering the treatment in an intensive, high-effort manner.

LSVT LOUD is built on three key strategies to make the treatment as effective as possible:
TARGET:  Vocal Loudness (amplitude)

These strategies are consistent with the most effective, evidence-based strategies for learning and neuroplasticity (the brain's ability to change).

MODE:  Intensive Dosage and High Effort

Intensive and high-effort treatment that encourages people to achieve more by always working harder (e.g., “louder, longer”). Sessions are delivered one-on-one and tailored to your individual communication goals.

CALIBRATION:  Generalization

"Recalibrating" sensory feedback so people recognize their new, louder voices are within normal limits. This increases the likelihood that people with PD will feel comfortable with and use their louder voice at home, work and play.

In addition to these key strategies, LSVT LOUD keeps people highly engaged, not only with frequent treatment sessions but also daily assignments for practicing newly learned skills at home and in their communities. Further, LSVT LOUD individualizes treatment exercises to each person’s interests and personal goals for improving communication.

These factors add up to a highly effective program that improves communication in everyday life as summarized in this 2015 research review.

Traditional medical treatments, such as pharmacological and surgical approaches, do not consistently or sufficiently improve speech for people with PD or atypical parkinsonisms, but many published research studies and countless clinical cases show that LSVT LOUD speech treatment works!

To understand how LSVT LOUD can help, it's good to first know how Parkinson disease (PD) affects communication. The most commonly reported voice and speech issues are: soft voice, hoarseness, monotone, mumbled speech, and rapid speech.

PD also affects communication in two other important ways:
  • Creating a mismatch between how you think you're talking and how you actually sound:
    • your voice may feel normal to you, when you may actually be speaking quietly,
    • when you are speaking louder as taught in treatment, it may feel too loud to you while friends and family may say you sound normal;
  • Interfering with thinking skills and coming up with the words you want to say.

Combined, all of these issues can make it difficult to feel confident in your communication. The good news is that something can be done! LSVT LOUD directly addresses these symptoms for proven communication improvements.

What LSVT LOUD Improves

LSVT LOUD improves communication by helping you speak with a voice and volume that's easy for others to hear and understand. The treatment is built on tailored exercises that strengthen the voice box (larynx) and speech system.

The main focus of the treatment is to “speak LOUD!” However, it doesn't teach you to shout. Instead, LSVT LOUD retrains the feedback mismatch that PD often causes, helping you develop and become comfortable with a voice that you can use without strain at a loudness you'll know sounds normal to friends and family.

Another key is working with an LSVT LOUD Certified speech-language pathologist whose professional skills and training will ensure that you improve your voice in a healthy way.

While LSVT LOUD focuses on vocal loudness, research shows the treatment can also help with other qualities that improve communication, including articulation and intonation and even facial expression.

Who LSVT LOUD Can Help

LSVT LOUD was created to address issues from Parkinson’s disease but also can help people with other conditions. Outcomes for various conditions and circumstances are explained in more detail below. The research evidence for LSVT LOUD for people with PD is very strong with three randomized clinical trials. In relation to other neurological conditions, evidence is emerging with single-subject and small group research. Ultimately, the LSVT LOUD Certified Clinician is the best person to determine if treatment could help.

You can still do the things you want to do and communicate with your loved ones as you'd like to.
People with mild to moderate PD typically see improvements in loudness, clarity and intonation. For many, using their improved voice becomes almost automatic — something they don't have to think much about during normal conversation — and improvements typically last 6 months or longer without any further treatment. These outcomes are well documented in research.

People with more advanced PD (for example, Hoehn & Yahr stages 4 – 5) may also see improvements in loudness and clarity of speech, though possibly to a lesser degree. They may also need periodic reminders from friends and families to “speak louder” and may need more frequent "tune up" treatment sessions. These outcomes are supported by research and our extensive clinical work.

People with dementia or other cognitive impairments may face the challenge of difficulty remembering to use the tools learned in treatment, but improvements are still very possible! A single cue (speak LOUD), repetition and intensity in practice, and using shorter sentences in treatment all help people with cognitive impairments learn. Many improve their loudness and clarity but may need friends and family to remind them to “speak louder.”

People with atypical parkinsonism may see outcomes ranging from improved functional conversation to improvement in key functional phrases (like “good morning” or “thank you”) with loudness cueing from caretakers and family members, which can still offer enormous benefit:

  • Giving someone with advanced-stage disease some access to spoken communication can be a great gift at that stage of progression;
  • The single, redundant cue of “loud” can greatly help patients with cognitive impairment;
  • Even if relying on external cueing, many people with atypical parkinsonism can improve voice and speech after treatment.

Children with cerebral palsy or Down syndrome can also benefit from LSVT LOUD treatment according to recent research. Clinicians gave the treatment to 25 children with cerebral palsy and nine children with Down syndrome and most parents reported immediate improvements, noting their children spoke more clearly and confidently and were more willing to talk with peers and teachers.   • Increased vocal loudness
  • Improved articulation and speech intelligibility
  • Improved intonation
  • Increased confidence with communication
  • Changes in neural functioning related to voice and speech

Speech challenges related to other neurological conditions may also benefit from LSVT LOUD. While most studies have examined outcomes for people with PD, early research shows that LSVT LOUD can help adults with speech issues arising from multiple sclerosis, stroke and ataxia, for example.

In summary, LSVT LOUD is the global standard for speech treatment for people with PD. Beyond PD, it has potential application to speech disorders in a range of other neurological conditions in adults and children. Ultimately, an LSVT LOUD Certified Clinician is the best person to determine if treatment could help you and to determine the best course of action.

After LSVT LOUD Treatment

The standard LSVT LOUD treatment program spans four weeks of individual clinical sessions (four times per week) plus daily homework and carryover exercises. Once that initial treatment is over, you'll continue to practice at least once a day for 10–15 minutes.

When is the Best Time to Start?

It's important to recognize that people with Parkinson's disease may not be overly aware of initial changes in their voice, speech and communication. This is likely due to real sensory changes in people with PD that cause the brain to not accurately register how softly they are actually speaking. If you've been diagnosed with PD, in addition to your perception of your voice, it's a good idea to also ask family and friends if they've noticed changes in your vocal loudness, voice quality or everyday communication.

That said, even if your speech is fine or does not feel impaired, it's never too early to begin LSVT LOUD treatment! When PD or other neurological conditions are first diagnosed, significant changes in the brain have already happened. Treatment with LSVT LOUD before noticing changes in communication may help you pre-empt problems and keep your voice and speech strong for years to come.
